Question:
How and when are the unnecessary files in the RefCacheFolders deleted?

Answer:
Tekla Structures deletes the files under RefCacheFolders only when you add a new version of a reference file with the same name as an existing one. Then the old cache file is removed.
Otherwise the reference cache files remain in the folder. Currently we do not have any tool for finding and deleting other unnecessary cache files automatically.
You can of course delete them manually. There is no risk of loosing data, because Tekla Structures rewrites the cache files when you open the model next time.
